Information & New Patient Form

DUE TO CORONAVIRUS PANDEMIC- we will NOT be holding our walk-in hours. Please call the office to schedule an appointment during our  hours of 8am-7pm

Regular Office Hours

Monday – Thursday: 9:30am – 5:30pm

We Have Walk-In Hours for existing patients (currently not available due to COVID 19)
Our walk-in hours are meant to address acute, quick problems only.

Morning Walk-Ins: Monday – Friday 8:00am – 9:00am.

Evening Walk-Ins: Monday- Thursday 5:30pm – 7pm.

We also are available in the office on Saturday mornings;
please call after 8:00 am on Saturday to make an appointment.

After-hours (including Sundays) we are also available to help you. Just call us for assistance.

Is your child medically cleared for physical activity?

COVID-19 infection rates are decreasing and as vaccinations increase, we are looking forward to youth sports getting back to the field (or court, pool or pitch). Due to the pandemic, it’s more important than ever to get a thorough medical examination and heart check before your children become physically active, especially if your child has tested positive for COVID-19.
